• New Youth Application: English and Spanish

• New Unit Application

• Activity Consent Form (required for BSA Aviation flights; recommended for all activities)
• Informed Consent Agreement (Permission Slip)

• Annual Health and Medical Record Form (entire form)
  • Parts A&B - Fill out annually, includes consent, hold harmless agreement, and talent release
  • Part C - Physical Exam for events over 72 hours in length
  • Part D - High Adventure

All tour and activity plans are done online. Please go to MYSCOUTING.ORG and sign in. Once you are signed in, go to Unit Tools on the left side of the page. Under that you will see a link for Tour and Activity Plans. Click on that and follow instructions to complete the form. Thank you.
