Online Forms
Some forms can be completed online, others can be filled out online, printed, and then
submitted. As they are available, they will be listed here.
- Online National Tour Permit
- Use the National Tour
Permit Online Assistant, to help assure you complete the form correctly. A National Tour
Permit is required whenever you travel more than 500 miles from home, or to Canada or any
other foreign country. If youre traveling less than 500 miles from home and
staying in the United States, you should use a local tour permit or
the online tour
permit.
- Common errors on National
Tour Permits
This is a list of the most common errors we have seen on National Tour Permits. As these
permits are approved by the Region and not by Gamehaven Council, please look specifically
for this information and assure it is on your permit. Also assure the permit is legible.
Doing this will speed up approval of the permit.
Mileage Round trip: This is often missing or incorrect. Most commonly, either
one-way mileage is listed, or the mileage given does not match that listed on the
itinerary.
Accident insurance in force for the unit: If "yes" is checked, the company
and policy number must be provided.
- LEADERSHIP AND PERSONNEL
Adult leadership information: The age of the leader and expiration date of the
leader's Scouting position is often missing.
Adult leader signature missing. The leader must sign to acknowledge he/she has a copy
of the Guide to Safe Scouting and that he/she read it. You may purchase a copy of
the Guide to Safe Scouting from the Council office, or download a copy of it from
the national scout site at www.scouting.org/pubs/gss/
Total number of participants in party is not provided.
- INSURANCE - For rental cars:
Enter the rental car company name and type of car in the block entitled "kind,
year and make of vehicle."
Instead of providing the owner's name, supply the name of driver, along with his/her
driver's license number.
Provide the passenger capacity of the rental car and insurance coverage of the driver's
insurance policy.
- TRANSPORTATION
When traveling by public carrier, both boxes 7A and 7B must be checked.
- HEALTH AND SAFETY
If box 8a is marked (swimming, boating or climbing/rappelling are included in the
program), then at least one adult must be trained for that activity and his or her
training information must be listed in the table below box 8a. You can take Safety Afloat
and Safe Swim Defense training online.
If training in Safety Afloat or Climb on Safely is required, list the individual who is
trained in CPR in the table below box 8a.
- ITINERARY
This section frequently is not completed correctly. Mileage should be shown for
each day. If different modes are used on the same day (e.g. private vehicle and then air),
please also break down the daily mileage by each mode. For example, you might show
Monday's mileage as 75 miles by automobile, and then 400 miles by public carrier (bus).
- SIGNATURES REQUIRED
The completed form must have three different signatures: (1) chairman of committee,
(2) tour leader, and (3) Scout Executive. If the tour leader is also the chairman of the
committee, then another person needs to sign in place of the chairman of committee. For a
Council contingent, only two signatures are required, the signature of the tour leader and
the Scout Executive.
